The Cadence Health team seeks a Nurse Practitioner to serve as the point person to help patients navigate their remote care journey. Our Nurse Practitioners are responsible for appropriately triaging patients based on vitals, and managing treatment plans and medications in collaboration with the patient’s physician/ provider.
You will also be in charge of making sure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system. You will support internal growth efforts to help Cadence scale exceptional care delivery to patients with CHF and other chronic conditions including, but not limited to hypertension, and Type 2 diabetes. You will be instrumental in shaping the culture of one of the fastest-growing teams at Cadence.
WHAT YOU’LL DO:
In our program, the Clinical Team Lead will dedicate 60% of their time to performing the duties of a Nurse Practitioner, and 40% of their time to leading and supporting other Nurse Practitioners. (this is subject to change based on the needs of the team and the company).
Cadence NP functions:
- Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with our team of Clinical Navigators, RNs, and MDs.
- Manage the patient's treatment plan and medications – with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for T2D and Hypertension.
- Respond to patient escalations identified via RPM – i.e. abnormal vitals, symptoms and labs.
- Lead regular virtual check-ins with patients to review labs and GDMT optimization and adherence.
As the Clinical Team Lead, you will:
- Perform all functions of an NP at Cadence.
- Help ensure all clinical quality standards and compliance targets for your team are met by conducting the quarterly audits, recommend process improvements initiatives, provide coaching, training and input on performance.
- Identify gaps in compliance and recommend remediation plans.
- Provide day-to-day leadership to the team.
- Lead Bimonthly team meetings.
- Will assist with new clinic assignments and licensure.
- Monitor a variety of databases and data sources daily and weekly as trends change.
- Review OKRs with team and how they are performing individually and as a team.
- Identify and coach performance issues on the team.
- Flexibility to work across time zones to assure coverage for the team Serves as a field clinical expert and resource to trouble-shoot problems that might be identified within the team or process and escalate to Leadership in a responsive and timely manner.
- Foster and promotes teamwork and a culture of inquiry, collaboration and learning.
- Assists in communicating policies and procedures, and pathways of care.
- Responsible for securing coverage for Call offs, approving PTO, holiday coverage and CME for their team.
- Will assist with interviewing potential candidates to the team.
- Work with the RN Team Lead to ensure adequate coverage for the entire clinical team as needed.
- Contributing to the growth of the Company through a successful team and have ownership of additional duties assigned.
WHAT YOU'LL NEED:
Education and/or Experience:
- 5+ years Nurse Practitioner experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high performing medical center.
- Master’s Degree as a Nurse Practitioner with willingness to expand state licensure as Cadence adds new markets and partners.
- Board certification and active license required.
- Multi-state compact licensure (RN) preferred.
- 3+ years of people management experience
Knowledge and skills:

- Experience working in a CHF bridge clinic environment, T2D clinic (preferred).

- Bilingual (preferred).

- Expected compensation range: $140,000-$150,000 annual salary
- Location: Remote, working West Coast time zone hours
