Integral Health is expanding its Behavioral Care Manager team to support primary care practices through the Collaborative Care Model.

As a Behavioral Care Manager, you will provide direct, patient-facing behavioral health therapy while coordinating care with primary care providers and psychiatric consultants.
This role combines short-term, evidence-based therapy with behavioral health care management. You will assess patients, develop focused treatment plans, provide brief therapeutic interventions, monitor measurable progress, document clinical care, and collaborate with the patient’s broader healthcare team.
This is not a care-management-only position. It is designed for a licensed clinician who enjoys providing therapy but prefers focused, measurement-based interventions over maintaining a traditional, long-term psychotherapy caseload.

- Provide direct, patient-facing behavioral health therapy through telehealth
- Deliver brief, evidence-based interventions using approaches such as CBT, motivational interviewing, problem-solving therapy, and behavioral activation
- Screen and assess patients for mental health conditions, substance use concerns, safety risks, and social determinants of health
- Use measurement-based tools such as the PHQ-9, GAD-7, C-SSRS, and SDOH screenings to guide treatment and monitor progress
- Develop patient-centered care plans with focused, measurable SMART goals
- Monitor symptoms, treatment progress, medication adherence, side effects, and treatment effectiveness
- Work toward clinically meaningful symptom improvement and adjust interventions when patients are not progressing
- Support PCP-led medication management by communicating relevant patient information to primary care providers and psychiatric consultants
- Assess safety concerns, provide appropriate crisis interventions, and escalate higher-acuity cases
- Determine when a patient’s needs exceed the short-term Collaborative Care Model and coordinate transitions to longer-term or specialty care
- Use a clinical registry to track patient follow-up, outcomes, outreach, and re-engagement
- Document therapy sessions, assessments, progress, and treatment recommendations in the electronic health record
- Complete relapse-prevention and transition plans for patients approaching remission
- Educate patients and families about diagnoses, symptoms, treatment options, and available resources
- Coordinate behavioral healthcare with primary care providers, psychiatric consultants, and other service providers
- Participate in regularly scheduled caseload consultations with psychiatric consultants
- Employees are expected to work between 31 and 40 hours per week, depending on organizational needs and patient demand
- Approximately 80% of working time will be devoted to patient care, with approximately 20% dedicated to documentation, care coordination, registry management, meetings, and team consultations
- An average of two evening shifts per week is required to support patient accessibility
- Schedules provide flexibility while maintaining consistent patient coverage, continuity of care, and team collaboration
- Overtime must be approved by leadership in advance
- Maintain a private and secure workspace suitable for confidential telehealth sessions
- Conduct all patient interactions in an environment that protects patient privacy and complies with HIPAA requirements
- Maintain reliable internet connectivity
- Be comfortable using telehealth platforms, electronic medical records, clinical registries, and other digital tools
- Master’s degree in a relevant behavioral health field
- Active, unrestricted New York LMSW, LCSW, LMHC, or LMFT license
- Ability to maintain an active license in good standing throughout employment
- At least two years of experience providing mental health assessments and direct therapeutic interventions
- Experience managing a behavioral health caseload with appropriate independence
- Ability to maintain a consistent full-time schedule, including an average of two evening shifts per week
- Experience delivering evidence-based interventions such as CBT, motivational interviewing, problem-solving therapy, or behavioral activation
- Experience using clinical tools such as the PHQ-9, GAD-7, and C-SSRS
- Strong clinical documentation, verbal communication, and written communication skills
- Comfort using EMRs, clinical registries, telehealth platforms, and other technology
- Ability to work independently while collaborating closely with physicians, psychiatric consultants, and the broader care team
- Ability to manage competing priorities and adapt as clinical and organizational needs evolve
- Experience providing behavioral health treatment to adolescents
- Knowledge of or experience with the Collaborative Care Model
- Spanish-speaking or bilingual
- Experience with population health or value-based care
- Experience working remotely
- Experience with healthcare startups
- Knowledge of SDOH interventions and community resources
- Familiarity with chronic illness, palliative care, end-of-life counseling, or medical terminology
