About Us

As a Behavioral Care Manager, you will provide direct, patient-facing behavioral health therapy and coordinate care with primary care providers and psychiatric consultants through the Collaborative Care Model.

This role combines short-term therapy with care management. You will conduct clinical assessments, provide brief evidence-based interventions, monitor measurable progress, complete clinical documentation, and coordinate treatment with the patient’s broader healthcare team.

This is not a care-management-only position. You must be comfortable providing direct therapy to patients, including adolescents, as a core responsibility of the role.

What You’ll Do

  • Provide direct, patient-facing behavioral health therapy to adolescents and adults through telehealth
  • Deliver short-term, evidence-based interventions using approaches such as CBT, motivational interviewing, problem-solving therapy, and behavioral activation
  • Conduct clinical assessments and develop focused, measurable treatment goals
  • Assess and respond appropriately to safety concerns, including completing C-SSRS screenings and escalating higher-acuity cases
  • Determine when a patient’s needs exceed the short-term Collaborative Care Model and coordinate an appropriate transition to longer-term or specialty treatment
  • Work closely with primary care providers and psychiatric consultants to coordinate behavioral healthcare
  • Screen for and monitor depression, anxiety, substance use, and other conditions using tools such as the PHQ-9, GAD-7, and C-SSRS
  • Track patient progress and adjust interventions when patients are not improving
  • Support PCP-led medication management by tracking adherence, side effects, and effectiveness
  • Use a patient registry to manage follow-up, outreach, and patient re-engagement
  • Document therapy sessions, progress, risk assessments, and treatment recommendations in the EHR
  • Complete relapse-prevention and transition plans for patients approaching remission
  • Educate patients and families about diagnoses, symptoms, and treatment options
  • Participate in regular caseload consultations with a psychiatric consultant

  • Pay: $45–$65/hour (based on experience & licensure) + up to 10% annual performance bonus, 5% paid biannually.
  • Paid for ALL Your Time: We pay for every minute you work with us — including but not limited to direct patient care, documentation, training, meetings, and professional development. Even if a patient no-shows, you are paid for the outreach/care coordination time.
  • Schedule: Set a consistent weekly schedule that works for you and your patients. Contractors must maintain at least 15 hours of recurring availability each week and provide continuity for their assigned patients. Evenings & weekends are required for this particular adolescent therapy role.

You Have

  • Master’s degree + active license in New York (LMSW, LCSW, LMHC, LMFT, or equivalent) — associate or independent licensure welcome
  • At least two years of full-time-equivalent, direct mental-health treatment experience, including experience providing individual therapy. Case management, intake, crisis response, or care coordination experience alone does not satisfy this requirement. The two years must be after obtaining a license.
  • Ability to commit to a minimum of 15 hours per week
  • Familiarity with evidence-based approaches like PST, MI, CBT, and Behavioral Activation
  • Comfort with EMRs and tech tools
  • Prior remote work experience
  • Spanish Speaking/ Bilingual a plus
  • Knowledge of the Collaborative Care Model (preferred)

  • You are unable to maintain at least 15 consistent hours of availability each week

  • You are uncomfortable managing clinical documentation, follow-up outreach, and multiple patient priorities
